change request form
- before 48 hours — no penalty for request change
- within 48 hours until 24 hours before appointment start time — 25% of service total due in order to accept reschedule
- 24 hours until day of appointment— 50% of service total due in order to accept changes or reschedule
- day of appointment — with APPROVAL ONLY 75% of total service booked due in order to accept changes or reschedule.
- no call no show will result in 100% of total service due x no longer acceptance of appointments.

all appointments require a card on file - no exceptions.
invoices will be sent out at the end of business day on the day of the scheduled appointment.
invoices must be paid in full in order to request another appointment
